Dr Philip Connell may collect and hold the following personal data about you: your name, address, email address, phone number, date of birth, gender, details of next of kin/emergency contact information and medical records (which will include details of your health, including any medical conditions and sex life/sexual preferences to the extent this relates to any symptoms/clinical treatment we are discussing with you). Dr Philip Connell uses your personal data to provide our medical services to you, comply with our legal obligations and to send you details of available appointments and appointment reminders by your preferred contact method. Dr Philip Connell uses the personal data set out above to provide our medical services to you in accordance with the contract to provide medical services which we have with you (or to provide information about our services in contemplation of your entering into a contract with us) and to comply with our legal and regulatory obligations as a provider of medical services. Dr Philip Connell is a registered medical practitioner subject to an obligation of professional secrecy.
Dr Philip Connell may contact you by email (or post if you do not have an email address or do not wish us to use you email address for this purpose) to send you details of available appointments, reminders to book your regular follow up appointments and related information about our practice and the medical services we provide to you. Dr Philip Connell may contact you with relevant updates on the basis of our legitimate interests of developing our relationship with our patients, promoting our services and growing our business. Where you receive these updates by email, you can unsubscribe from receiving these by following the “unsubscribe” link contained in each email we send to you. You also have a right to object to us sending you updates (by email or post), please see below for further information.
You have a right to object to Dr Philip Connell sending you updates. If you do object then we will not be able to send you details of available appointments, reminders to book your regular follow up appointments or related information. This may mean that you find it harder to find available appointments, miss out on suggested health and wellbeing actions and do not hear about new services we offer from time to time.
Dr Philip Connell does not transfer any of your data outside of the European Economic Area.
Dr Philip Connell will share some of your personal data with selected third party processors in order to provide our medical services to you. This will include our IT service provider, our email platform provider and our document shredding provider who will process your personal data on our behalf.
Dr Philip Connell may also share some of your personal data with selected healthcare/wellbeing professionals, specialists and your NHS GP, with your consent. If, following consultation with you, Dr Philip Connell feels you may benefit from receiving the services of any third party healthcare/wellbeing professional, e.g. surgeons, nutritionists, dentists, physiotherapists etc., Dr Philip Connell may (with your consent) share some of your personal data with such third party as a referral so they can contact you about the services they may be able to provide to you. Dr Philip Connell will, on your request (and with your consent) share some of your personal data with your NHS GP. Where Dr Philip Connell shares your personal data with a third party who provides associated health/wellbeing services and/or your GP they shall also become a data controller in respect of your personal data and you should review their privacy policy to ensure that you are comfortable with their use of your personal data.
Dr Philip Connell may also be required by law or regulation to share your personal data with HM Revenue & Customs and other regulatory authorities although this will not include any of your medical records unless we notify you otherwise.
Dr Philip Connell will retain and process your personal data in accordance with industry guidance. Dr Philip Connell will continue to hold and use your personal data as set out above for such time as you are a patient at our practice (to enable us to provide our medical services to you during this period) and thereafter will continue to hold these for record keeping purposes for up to ten years after your death; to ensure we comply with our legal obligations, good industry practice and to assist us in defending any potential claims.
Dr Philip Connell have put in place appropriate security measures to protect your personal data from being accidentally lost, used or accessed in an unauthorised way, altered or disclosed. In addition, Dr Philip Connell limits access to your personal data to those employees, agents, contractors and other third parties who have a business need to know such information. These parties will only process your personal data on our instructions and they are subject to a duty of confidentiality. Details of your medical records and your other personal data are stored in a locked electronic and/or physical file to which only Dr Philip Connell has access.
You have the following rights in respect of the personal data Dr Philip Connell holds about you: (i) a right of access to such data; (ii) a right to object to our processing for direct marketing purposes; (iii) a right (in certain circumstances) to request that your personal data be transferred to you or a third party; and (iv) a right (in certain circumstances) to have inaccurate personal data rectified, blocked, erased or destroyed. You also have a right to lodge a complaint with the Information Commissioner’s Office (ICO) but we request that you contact us before escalating any complaint to the ICO.
Dr Philip Connell may update this policy from time to time to reflect changes in the way we use your personal data or legal/regulatory requirements. If we do update this policy, Dr Philip Connell will let you know by sending a copy of the updated policy to you.
